A beauty, most of the time

Pros ergonomic; attractive,
Cons flex can be a problem; grime build up
Recommended it? Yes
The Bottom Line:  Vast improvement over the 'puck' mouse
This was my first Mac mouse (I switched over on the Mac's 20th anniversary!) and it was all quite new to me. I was familiar with plugging it into the keyboard and this makes up for it being rather a short cable (a couple of feet) and this is fine providing you keep your mouse close to the keyboard (why else would it be wandering away from its friend, anyway?) The actual cable could do with being a bit more flexible, as it tends to get 'stuck' in one position, coiled around your desktop space (i've seen apple mice with lots of nasty loops in the flex). I'm getting used to this aspect, and the short cable prevents the flex from getting out of hand.
Ergonomically, this is easily the best mouse i have used, clicking the whole mouse down produces a satisfying 'click'. It's smooth, all clear plastic body (i'm sure you will have seen this) is nice to hold and i don't see that having too small / too large a hand would prove a problem. The one button Apple feature isn't too much of a bother for an ex PC user, and i am more than used to not having a scroll wheel to scroll down web pages now. Having to press CTRL to produce the effect of a right click feel natural, as i used to find extensive right clicking became quite uncomfortable. Also, it would lose its lovely appearance if it had a second button. That said, playing games like Quake III, you will not be able to quickly scroll to select new weapons etc like your windows friends can, but then the Mac is not really a gamer's platform anyhow. The mouse does look great, if a bit more cheap looking than you see in photos.
The troubles concern dirt and grime. After an hour or so use, you find a grimy residue over the clear plastic that you have to clean off with your sleeve every so often. I get quite obsessive, as you can feel if there is any grime on the mouse surface and it also looks quite noticeable (like mud splats on a pristine white car). i clean mine when guests come round to keep the illusion of its perfection alive.
More disconcerting is when grime gets under the mouse. Instead of the smooth glide, you get a scratchy effect which is not so nice to use. I have to wipe around the rim of the bottom to keep a smooth action, and make sure my desk is free of crumbs etc. It does work on any surface, though and often i use it one my jeans if i'm lazy.
The grime factor is probably minor, and i have not used a mouse where a grime build up has not been a factor, so i would give this the thumbs up. Having used the previous Apple 'puck' mouse on friend's computers, i can say that this is a HUGE improvement over that menace.
